MEMBERSHIP APPLICATION

A three-month commitment is required when signing up for a continuous membership to the Cedar Valley SportsPlex; therefore, monthly membership dues must be withdrawn from your account on the 20th of every month for a MINIMUM of three months. If you choose to terminate your membership after those three months, a completed membership termination form MUST be received 30 days prior to the cancellation date, in order to the withdraw from your bank account or debit/credit card.

A guardian over the age of 18 must be in the facility at all times with children under the age of 10.

In order to qualify for our senior benefits, you must be at least 62.

MEMBER GUEST PASS: We encourage our members to bring guests to enjoy all the Cedar Valley SportsPlex has to offer. We ask that members stop by the front desk to purchase a pass for their guest(s). Daily member guest rates are above. Members may also purchase a Member Guest Punch Passes for $60, which includes 10 entries into the Cedar Valley SportsPlex or for $30, which includes 5 entries into the Cedar Valley SportsPlex.

Members 18 years or older must purchase the Member Guest Punch Pass in person. Member Guest Punch Pass must be paid-in-full at time of purchase.
